Cox Care Mobile - Oct. 11

The Cox Care Mobile is scheduled to be at Clever schools on October 11, 2018.  The Care Mobile is a medical office on wheels that provides FREE services to children ages birth to 18 years. This service is provided whether you have insurance or not.  Please note:  The Care Mobile does charge for sports physicals to those that do not qualify for free or reduced lunches.  You may call the school nurse to inquire about the services provided, to schedule an appointment, and to receive the necessary paperwork.  Thank you!

SPORTS PHYSICALS:
-Students who qualifies for free or reduced lunches or no insurance- no charge for sports physical
-Students who does not qualify for free or reduced lunches- $25 charge for sports physical

IMMUNIZATIONS:
-Students without insurance or those with active Medicaid can receive vaccines for Free.
-Students with private insurance can receive vaccines and the cost of service will be submitted to the insurance company.  ***Insurance must be accepted by Cox Health***

End of the Year Information!!!

Parents,
We have several things going on next week. It’s hard to believe that the 2017-18 school year is coming to a close. We will miss our students, but we are so proud of how much they have put into learning this year.

Monday
  • Our read-a-thon will be celebrated! If your child completed his/her reading journey, he/she may bring a blanket, a drink and a snack to celebrate the accomplishment.
Tuesday
  • Field day will be at the high school. We will be walking on the trail over to the high school and participating in planned events on the track.
  • Please make sure your child is wearing tennis shoes for running, comfortable clothes, and has applied sunscreen. We will be in the sun for several hours.
  • Sack lunches may be purchased at your usual cost from the cafeteria for this event. If your child chooses to bring his lunch, please put it in bags or sacks that can be completely tossed after they have been eaten. This makes items easier to transport and dispose of afterward.
Wednesday
  • School will dismiss at noon. Sack lunches will be available for purchase at the cafeteria.
  • Summer School placements will be posted Wednesday, May 23 at 4:00 P.M.
  • 5th Grade student placements will be posted August 1.

Where:  South Gym
Cost: $20 includes a T-Shirt

Please turn in registration form and payment to your child’s teacher (1st-4th), or the Upper EleMiddle office (5th-6th) no later than April 27th .  Make checks payable to Clever Booster Club.

Pick up will be at the South Gym/Upper EleMiddle circle drive at 5:00.  Please be prompt in your pick up time.

Science Letter from grade card note

Parents,


We are making plans to start some science investigations, soon! Many of our
Science standards require “planning, conducting, and designing” certain
objects. In order to do this, we need your help.


We need some classroom supplies such as: black expo markers, pencil top
erasers, cheap toothbrushes (1 per student), empty toilet paper and paper
towel rolls, empty water bottles or 20 oz soda bottles with the lids, milk
jug LIDS only, pringles can with lid, balloons, rubber bands, straws, wooden
skewers, full size hot glue sticks and anything else that would resemble
these items.


We couldn’t do this without you, and we can’t wait to see what the kids
come up with!

Cox Care Mobile - April 16, 2018

Does your child need vaccines for the 2018-2019 school year?  If so, your child can receive the required vaccines on the Cox Care Mobile.  Students who are uninsured, under-insured, or have Medicaid can receive vaccines at no charge.  Private stock vaccines will be available for those with insurance.  Insurance will be billed for immunizations given.  The Cox Care Mobile will be at Clever schools on April 16, 2018.  Call your school nurse today to schedule an appointment.

Cox Care Mobile Update

**UPDATE**

The Cox Care Mobile has pushed back the start date for the charging of sports physicals until April 1, 2018.  
Beginning April 1, 2018, the Care Mobile will be billing for certain services:

Sports Physicals-
  • Student who qualifies for free or reduced meals, no insurance- No charge
  • Student who does not qualify for free or reduced meals, no insurance- $25.00 charge
The Care Mobile is scheduled to be here on March 20, 2018.  For the March visit, your child can still receive a sports physical free of charge.  Call today to schedule an appointment.  Thank you!

Dear Parent or Guardian,
We are pleased to announce the Tooth Truck has the opportunity to treat eligible children at our site in the coming weeks. The Tooth Truck provides comprehensive treatment, from cleanings and x-rays, to repairing cavities at no charge for qualifying families.  Requirements for being seen and treated on the Tooth Truck are:

-Qualify for the free and reduced lunch program at school
-Cannot be an established patient of a local dentist
-Cannot have private dental insurance
-Must complete a Tooth Truck application form

The Tooth Truck treats Medicaid-eligible and uninsured children from ages 3-18 years. Children seen last year are not eligible for a routine check up but should apply if there is a new dental concern.  
You may call your school and request an application form.  Please complete an application form and return to the school nurse as soon as possible.   
The Tooth Truck staff will be scheduling students very soon.
